Sabra respects your privacy rights. This Privacy Policy applies only to the Site and not to Personally Identifiable Information collected from investors or employees outside of the Site.  Except as set forth in this Privacy Policy and our Terms of Use, we do not release Personally Identifiable Information about our users without their permission.

What Information this Privacy Policy Covers

Certain of our Site’s functionality can be used without revealing any personal information, though for some specific features or services, personal information may be required. For example, submitting a request to have information mailed to you. In order to access certain features and benefits on our Site, you may need to submit “Personally Identifiable Information” (i.e., information that can be used to identify you). Personally Identifiable Information can include information such as your name, home address, home telephone number and home email address. You are responsible for ensuring the accuracy of the Personally Identifiable Information you submit to Sabra.

What You Consent to by Using Our Site

Please understand that by submitting any Personally Identifiable Information to us, you consent and agree that we may collect, use and disclose such Personally Identifiable Information in accordance with this Privacy Policy and our Terms, and as permitted or required by law. If you do not agree with this Privacy Policy, then please do not provide any Personally Identifiable Information to us. If you refuse or withdraw your consent, or if you choose not to provide us with any required Personally Identifiable Information, we may not be able to provide you with the services offered on our Site.

Information We Collect

We gather two types of information about users through the Site:

1. Information Collected Automatically. When users come to our Site, through the use of third-party analytic tools provided by Google Analytics, we may track, collect and aggregate Non-Personal Information indicating, among other things, which pages of our Site were visited, the order in which they were visited, and which hyperlinks were “clicked.” Collecting such information may involve logging the IP address, operating system and browser software used by each user of the Site. Although such information is not used by us to identify you, Google Analytics may be able to determine from an IP address a user’s Internet Service Provider and the geographic location of their point of connectivity.

These analytic tools may also use or may use cookies to help determine, without limitation, the type of content and sites to which a user of our website links, the length of time each user spends at any particular area of our website, and the specific functionalities that users choose to use. Essentially, cookies are a small text file that is stored on your computer and serve as a user’s identification card for the Sabra servers. Cookies allow Sabra to serve you better and more efficiently, and to personalize your experience at our Website.  We do not use cookies to retain Personally Identifiable Information about you.

Most browsers offer instructions on how to reset the browser to reject cookies in the “Help” section of the toolbar. If you reject our cookies, certain of the functions and conveniences of this website may not work properly, but you do not have to accept our cookies in order to productively use many functionalities of the website.

We do not track signals or track users across different websites.

2. Personally Identifiable Information Provided by You. We collect Personally Identifiable Information that you provide to us when you send us email messages and/or request information on our Site. We will use the Personally Identifiable Information that you provide to respond to your questions, send alerts or provide you with the specific information you request. The Site features an email notification service provided by a third party, S&P Global Market Intelligence, to which you may subscribe. Sabra does not maintain or have access to information you may choose to provide to S&P as part of their email notification service.

Confidentiality & Security of Personally Identifiable Information 

We will use commercially reasonable efforts to keep your Personally Identifiable Information secure, and to protect your Personally Identifiable Information from unauthorized disclosure to or access by third parties. Please note that we do not guarantee the security of Personally Identifiable Information, and there is some risk that an unauthorized third party may find a way to circumvent our security systems or that transmission of your information over the Internet will be intercepted.

Disclosure of your Personally Identifiable Information

Sabra may share or disclose your Personally Identifiable Information for various purposes as noted below:

(a) Disclosure to Service Providers.  We use a variety of third-party service providers, vendors and contractors that perform functions and services on our behalf (collectively, “Service Providers”), including  providing us with technology, services, and/or content related to the operation and maintenance of the Site. We endeavor to limit Service Provider access to your Personally Identifiable Information as reasonably necessary for the Service Provider to provide its services to us.

(b) Government Authorities, Legal Rights and Actions. Sabra may share your Personally Identifiable Information with various government authorities and third parties in response to subpoenas, court orders, or other legal process or investigatory requests; to establish or exercise our legal rights or to protect our property; to defend against legal claims; or as otherwise required by law. In such cases we reserve the right to raise or waive any legal objection or right available to us. We also may share your Personally Identifiable Information when we believe it is appropriate to investigate, prevent, or take action regarding illegal or suspected illegal activities; to protect and defend the rights, property, or safety of Sabra, the Site, our users, customers, or others; and in connection with our Terms of Use and other agreements.

(c) Business Transactions. Sabra may sell, transfer, or otherwise provide some or all of its assets, including your Personal Identifiable Information, to an affiliate or a third party in connection with a merger, acquisition, reorganization or sale of assets or in the event of bankruptcy.

Sabra does not sell or share your Personally Identifiable Information with any third party for their direct marketing purposes.

Protecting Children’s Privacy

Sabra strongly believes in protecting the privacy of children. In line with this belief, we do not knowingly collect or maintain Personally Identifiable Information on our Site from persons under 13 years of age, and no part of our Site is directed to persons under 18 years of age. If you are under 13 years of age, then please do not use or access this Site at any time or in any manner. We will take appropriate steps to delete any Personally Identifiable Information of persons less than 13 years of age that has been collected on our website without verified parental consent upon learning of the existence of such Personally Identifiable Information.

Notice to California Consumers

If you are a resident of the State of California (“California Consumer”), you have additional rights under the California Consumer Privacy Act (“CCPA”).  During the past 12 months, we have collected the following categories of personal information from California consumers:

Identifiers such as name, home address, email address and phone number for those who us or request information regarding Sabra and investment updates.  We obtain that information from California Consumers who contact us or request such information or updates.  We may share such information with service providers, such as S&P Global Market Intelligence, to deliver investor updates. 

If you are a California Consumer, you have the right to request us to:

(a) Disclose the categories of personal information we collected about you in the preceding 12 months, categories of sources from which the personal information was collected; the business or commercial purpose for which we collected or sold the personal information; the categories of third parties with which the business we have shared your personal information, categories of personal information that we sold in the preceding 12 months, and for each category identified, categories of third parties to which we sold that particular category of personal information, add the categories of personal information that we disclosed for a business purpose in the preceding 12 months, and for each category identified, the categories of third parties to whom we disclosed that particular category of personal information (“Request to Know”);

(b) Disclose the specific pieces of personal information that we have collected on you (“Request to Know”);

(c) Delete personal information we have collected on you (“Request to Delete”); and

(d) Opt-out of the sale of the personal information we have collected on you (“Right to Opt-out”).

You can only make two Requests to Know in a 12-month period, and the information provided need only cover the 12-month period prior to receipt of your request.  We do not discriminate against any California consumer that exercises their rights under the CCPA.

You may submit Requests to Know and Requests to Delete by emailing us at investorrelations@sabrahealth.com. We will need to verify your identity to respond to Requests to Delete and Requests to Know. If your request concerns categories of personal information collected, we can request from you two data points of personal information to verify your identity. If your request concerns specific pieces of personal information, we can request from you at least three data points of personal information as well as a signed declaration with penalty of perjury to verify your identity.

As a California resident, you have the right to designate an agent to exercise these rights on your behalf.  We may require proof that you have designated the authorized agent to act on your behalf and to verify your identity directly with us. 

No Sale of Personal Information

We do not sell your personal information as defined under the CCPA or Nevada law.   However, if you want to submit an opt-out request, please email us at investorrelations@sabrahealth.com.

Changing Our Privacy Policy

Please note that we review our privacy practices from time to time, and that these practices are subject to change. Any change, update, or modification will be effective immediately upon posting on our website. We will notify you of any material change to this Privacy Policy by posting a notice on our Site’s homepage for a reasonable period of time following such update, and by changing the effective date (located at the bottom of this page). Be sure to return to this page periodically to ensure familiarity with the most current version of this Privacy Policy.
